Evidence for applicants who have a personal medical condition or disability
If you are applying for special circumstances because you have a personal medical condition or a disability, the evidence required is detailed below.
Application process
If you wish to be considered for special circumstances, you must complete all of the following steps:
- Ensure that you indicate that you wish to apply for special circumstances on your Oriel application form.
- Complete the following registration form. Please note, the form will only be available during the published application window.
- Download and complete the relevant special circumstances application form.
- Print and scan your special circumstances application form, along with all of your supporting evidence as a single document and email it to [email protected].
Supporting evidence
You must provide all of the following:
A fully completed Criterion 2 - Personal Medical Condition or Disability application form
This can be downloaded from this page.
Proof of your current address, dated within the last 3 months
Examples of acceptable evidence include:
- bank statement

- credit card statement
- driving licence
- mobile phone bill

Electronic documents downloaded from online banks or online utility providers will be accepted.
A report written by the current medical specialist treating your condition or Occupational Health physician
The report must:

- include the following information:
- a description of your current medical condition or disability.
- a description of your ongoing treatment and frequency, including what treatment is required, where this is delivered and how often you are required to attend hospital.
- an explanation of the reasons why your follow up treatment cannot be delivered elsewhere in the UK.
- the impact transferring your care elsewhere would have on you.
If your evidence has been received electronically and has not been signed or has an electronic signature, please also provide a copy of the email the evidence was attached to in the evidence pack.
Adapted housing
If you live in adapted housing that cannot be easily replicated elsewhere, a written statement confirming this from your GP or local council in written form, dated within 6 months of the application deadline, should also be provided.
Joint application evidence
Applicants who are undergoing fertility treatment together, and applying in the same recruitment round, can submit a joint application with a single set of evidence. You must both be named in the supporting evidence.
