A Fitness to Practise form must be submitted if you make a declaration on your application

If you make a Fitness to Practise declaration on your application form, you must complete a form and submit it to the Lead Recruiter providing further information. This must be provided at the time of application.

You should provide your supporting information form by email at time of application to the Lead Recruiter of the specialty to which you are applying to.

Further details on the nature of the declaration may be requested and should be provided prior to the deadline for submission of applications. Failure to provide this detail by the closing date may result in your application being rejected.
