Fitness to Practise

A Fitness to Practise form must be submitted if you make a declaration on your application

If you make a Fitness to Practise declaration on your application form, you must complete a form and submit it to the Lead Recruiter providing further information. This must be provided at the time of application.

You should provide your supporting information form by email at time of application to the Lead Recruiter of the specialty to which you are applying to.

Further details on the nature of the declaration may be requested and should be provided prior to the deadline for submission of applications. Failure to provide this detail by the closing date may result in your application being rejected.

For propriety, such declarations need to include all convictions, regardless of whether or not they can be regarded as 'spent'; this includes driving offences, with the exception of fixed penalty notices and parking offences.

This will be assessed from your application, at interview, via your references, and will be checked prior to you taking up any employment offered.

The application form will contain a full page dedicated to nine fitness to practise issues.

Should you answer yes to any of the 9 corresponding questions, you will be required to give further details. This information should be supplied using the designated form by the application closing date. The form should be emailed to the region managing your application using the fitness to practise email addresses given below.
