Certificate of Readiness to Enter Specialty Training (CREST) 2021 Dean’s supporting declaration

The Dean's Supporting Declaration proforma can used if you have not satisfactorily completed either a UKFPO-appointed 2-year Foundation programme or FY2 standalone post

This proforma is for use by applicants who have started but not satisfactorily completed either a UKFPO-appointed 2-year Foundation programme or FY2 standalone post. 

You should liaise with the Foundation School Director where your training took place, providing an explanation of how your training has been augmented and providing evidence as requested.  

After discussion with the Foundation School Director, this form should be submitted to the Postgraduate Dean where the foundation training took place (together with all evidence) for the Postgraduate Dean to complete and sign.

This should be submitted at the time of application.
